ขั้นตอนการสมัคร DofE

1. เลือกระดับรางวัลที่ต้องการ ทองแดง / เงิน / ทอง
2. กรอกข้อมูลส่วนตัวของนักเรียน
3. กรอกข้อมูลการติดต่อและประวัติสุขภาพของนักเรียน
4. กรอกข้อมูลผู้ปกครอง
5. กรอกข้อมูลยินยอมให้ใช้ข้อมูลและเรียกเก็บค่าลงทะเบียน
6. กดส่งข้อมูล พิมพ์ใบสมัคร และลงชื่อให้เรียบร้อย นำมาส่งที่ Ms Eden หรือ อ.วรทย์ ปิดรับสมัคร 12 มิถุนายน 2569

1. Select the Award Level you wish to apply for: Bronze / Silver / Gold
2. Complete all student personal information.
3. Complete the student’s contact details and medical information.
4. Complete the parent/guardian information.
5. Provide consent in the section “Consent and Registration Fee Agreement”.
6. Submit the application form, print the completed form, sign it, and submit it to Ms Eden or Aj. Warot.
